Job Description: Reporting to the Manager, the Assessor Coordinator - Degree is responsible for the assessment of clients, coordination and evaluation of Home Care services and case management of clients.  The Assessor Coordinator - Degree will ensure that the delivery of home care and case management services is consistent with the policies, vision, mission and philosophy of the Saskatchewan Health Authority.
Required Qualifications

A degree in nursing, social work, physical therapy or occupational therapy.
Registered with the appropriate professional licensing organization
Valid Saskatchewan driver's license and access to a reliable vehicle
Knowledge, Skills and Abilities

Ability to interact with or work in a team setting
Ability to organize, prioritize workload and meet deadlines.
Ability to problem solve and make decisions in a timely manner
Ability to work with a diverse group of clients
Able to work independently and plan and organize time efficiently
Communication skills
Demonstrated knowledge of community resources, internal and external to Home Care
Demonstrated skill in advocacy, negotiation and conflict resolution
Knowledge of a Client Centered Case Management Approach
Knowledge of Home Care philosophy, policy, eligibility criteria and services
Understands and is committed to the philosophy of Primary Health
Working knowledge of current technology and ability to perform computer tasks.
