Description

Through funding made possible by Saskatchewan Seniors Mechanism, the Town of Shaunavon has entered into an agreement to be a pilot project for Social Prescribing for Seniors.  This 3.5 year project will include the hiring of a half-time contract position (20 hours/week) to implement social prescribing for seniors in the community.

Job Summary:
To empower seniors to take control of their health and wellbeing through a holistic approach by connecting the senior population with community groups and services for practical and emotional support.  The Community Connector will assist seniors to find, access and/or create new and existing supports with local partners.

No specific experience necessary.  Preference to individuals who have worked with and are passionate about the aging population.  

Qualifications:
Ability to listen and empathize with individuals
Commitment to reducing health inequalities
Demonstrates personal accountability and considerable self-initiative
Knowledge of and ability to work to policies and procedures including confidentiality, safeguarding, information governance and Health and Safety
Data collection
Commitment to personal and professional development
Knowledge of the local community is considered an asset
Access to own transportation
Valid Class 5 Drivers license
Able to work independently and with different members of the community
Basic computer skills
Communication Skills
Organizational Skills
Creativity

Requirement to provide Criminal record check/Vulnerable Person Check.  Willingness to work flexible hours in order to meet work demands.  Access to own transportation and ability to travel on a regular basis.  This will be a home-based position with potential to access office and meeting space as necessary.
