General Description

Sturgeon Lake Health Center is seeking a Licensed Practical Nurse to join our dynamic Health team to assist in delivering professional, ethical, culturally sensitive, and holistic health care services to clients, families, and Sturgeon Lake First Nation.  
The Licensed Practical Nurse will assist with Community Health Program by providing support to Chronic Disease and Aboriginal Diabetes Initiative programs by providing nursing care, assessments, and care planning in conjunction with a RN, treatment, and the assignment of service hours for Sturgeon Lake First Nation. The Licensed Practical Nurse is a valuable member of a team, and together to will promote health and well-being for clients, and families.  

Duties Relating to Accountability
A.    The LPN is accountable for reporting, recording, working within program guidelines, and managing the workload of the assigned tasks. Duties in this area may include:
    Reporting any concerns about client safety or health risks to nursing supervisor.
    Reporting any changes in the client's condition/ situation to nursing supervisor.
    Participating in case management meetings.
    Carrying out assigned tasks.
    Maintaining confidentiality.
    Reinforcing program mission statement and philosophy to client, family, and community.
    Notifying supervisor when unable to complete assigned task.
    Submitting monthly statistics/ reports as required.
    Working within their scope of practice.

Qualifications:
    Graduate of an accredited school for Licensed Practical Nursing.
    Current license to practice and in good standing with SALPN.
    A minimum of one-year nursing experience.
    Maintain a valid Saskatchewan driver's license.
    Demonstrated ability to take direction and to work with others.
    Maintain CPR and First Aid Certificates.
    Previous experience working in a First Nation is an asset.
    Clear Criminal Record and Vulnerable Sector check.
