Phoenix Residential Society, a community based health care organization, has an opening for a full time position as Intensive Case Manager in our Housing and other Management and Engagement Services (HOMES) for maternity coverage until October 31,2024 with possibility of extension.

Working under the overall direction of the HOMES Supervisor, the Intensive Case Manager position is an integral part of a team-based approach in the provision of support services, referrals, advocacy, and system navigation to those individuals who are chronically homeless.  This position will focus primarily on intakes and assisting individuals experiencing homelessness with accessing housing for the first two months.
