Facility: Buffalo Narrows Health Centre
Job Description: Under the direction of the Manager of Home Care or designate, provides an accessible client centered and integrated approach to the processing of requests for Homecare services; performs initial consultation, preliminary assessment and identification of needs for potential clients; takes and screens referrals from various sources: determines eligibility of clients, initial interventions and the response time needed.

This position is a new position as such requires the successful candidate to identify improvements needed in existing process: and to use creativity and resourcefulness to help improve response time, and ensure appropriate and effective client care in the community. Integrating new and improved intake procedures into the present environment will be the key component of the position.
Required Qualifications

Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Valid Class 5 driver's license
Knowledge, Skills and Abilities

Ability and proven capacity to communicate, consult and work effectively with diverse groups of professionals, clients, and colleagues through written and verbal means
Ability to be self directed and exercise independent nursing judgment in a community-based environment
Ability to demonstrate strong clinical assessment, decision making, and critical thinking
Assesses the need for, develops, and recommends policies, procedures, protocols, clinical practice guidelines, supplies, and equipment relating to the care of this population, in coordination with the Head and Neck Cancer Surgery Medical Director, administrators, interdisciplinary staff,
