Required Qualifications

Bachelor Degree/Diploma in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Experience
1 year of experience with CPAP, BiPAP or ventilated patients with teaching/counselling related to the need of patient and discharge plan.
Knowledge, Skills and Abilities

Computer skills in such programs as Microsoft Office (Word, Excel, PowerPoint, Electronic Medical Record etc.)
Demonstrated ability to assess a clients readiness to learn and assist the client and family to integrate implications of chronic disease into their lifestyles using a self-care approach to chronic disease management
Demonstrated ability to establish and maintain effective interpersonal/consultative relationships and communication patterns with all contacts and with other health professionals of the interdisciplinary team
Demonstrated commitment to continuous learning through self-development continuing education programs and informal learning opportunities
Demonstrated cultural sensitivity and openness to working with diverse populations
Demonstrated individual and group facilitation, teaching/counselling skills, communication skills with patients/families/health professionals/students
Demonstrated initiative, self-direction, organizational and leadership abilities
Demonstrates an interest in Chronic Disease Management
Excellent verbal and written communication skills
Knowledge of hospital and community resources to ensure continuity of care
