Reporting directly to the Health Director and the STC Clinical Supervisor, the Prevention Program Family Support Worker will work within a team to ensure the development of effective prevention strategies and support programs to be delivered into an integrated case management model to support families and prevent children to coming into care. Working with families and the communities, measures will improve the quality of life of families and children while improving the capacity of members to provide quality level of care to children and families.
Duties:
Follow Mistawasis First Nation Personnel Policy as set by Chief and Council
Consult Elders for access to cultural and traditional supports where families indicate a desire.
Work with the FOF Home Visitors (Growing Great Kids) and assist in the delivery of the GGK curriculum by integrating it as part of the case management and referral process.
Develop individualized case plan and family support plans with each family.
Keep accurate and current documentation for each client
Qualifications:
Certificate or Diploma in a social/health science, human services discipline or related field
Knowledge of the Mistawasis First Nation traditions, values and customs and ability to speak the Cree language is an asset
A minimum of two years working with women (pre and post-natal) and families
Must have a valid Saskatchewan drivers license and reliable vehicle and meet community insurance requirements;
Must be eligible to receive a current, original Canadian Criminal Record Check (CPIC) and Automated Child Abuse Record check ;
