The Home and Community Care Certified Home Health Aide works under the general supervision of the Home and Community Home Care Registered Nurse. She/he uses her/his skills to effectively assist Home and Community Care clients to attain their optimum level of independent functioning in activities of daily living.
DUTIES:
to assist community members living with persistent and acute illness in maintaining optimum health, well-being and independence in their homes and communities;
Home Management Tasks:
- Assisting client to be as independent as possible;
- Light Housekeeping;
- Changing Linen;
- laundry;
- Menu Planning;
- Meal preparation/ assistance;
- Shopping Assistance;
- Assisting with clients personal business
to deliver home support personal care services:
- Assisting client to be independent;
- Daily hygiene assistance;
- Bathing and grooming;
- Assistance with eating/feeding;
- Assistance with transfers/mobility;
- Assistance with toileting;
- Dressing;
- Range of motion exercises;
- Basic wound dressing
a system of record keeping and data collection to carry out program monitoring, ongoing planning, reporting and evaluation activities.
Qualifications:
Health Care Aide Certificate or equivalent Special Care Aide/Continuing Care Assistant Certificate
Food Safe training
Current CPR , AED, and First Aid Certificates;
Valid Drivers License and reliable vehicle;
CPIC (Canadian Criminal Records Check) will be required upon successful job offer;
