Lac La Ronge Indian Band Health Services is inviting applications for 1 - Permanent Full Time, Community Health Representative (CHR) to assist in the Home & Community Care Program. The successful candidate will work in the community of La Ronge, under the supervision of the Home & Community Care Nursing Coordinator. This position aims to empower community members to make healthy decisions, through demonstrating, teaching and providing support and encouragement with the focus on addressing health needs of the community.  

Duties:
-    Assisting health staff in delivery of health care services and contributing to the health team through health advocacy, health promotion and illness prevention;
-    Act as a liaison between community members and LLRIBHS Staff and health programs;
-    Assist in the delivery of health programs by obtaining consents, transporting community members, booking Child Health Clinics, participating in community health fairs, events and activities;
-    Carrying out health promotion programs by planning workshops, in-service sessions, community education events and developing public awareness materials;
-    Work effectively with health care team;
-    Other duties as assigned by supervisor and/or designate;

Qualifications:
-    Community Health Representation certificate and/or a related health care certificate;
-    Basic Computer skills with knowledge of Microsoft Office 2003 including Microsoft Outlook;
-    Excellent verbal and written communication skills;
-    Ability to speak and/or understand Cree is a definite asset;
-    Excellent organizational skills; Ability to work within multidisciplinary team;

Salary/Wage:     $ 29,783.00 to $ 40,028.00 (depending on Education and Experience).
