Reporting directly to the Health Program Manager with clinical supervision provided by Saskatoon Tribal Council Focus on Families Program Coordinator, the Focus on Families Home Visitor responsible for carrying out the duties as outlined herein.
Duties:
1.Adhere to the Personnel Policy as set out by the Yellow Quill Chief and Council;
2.Maintain strict confidentiality guidelines at all times;
3.Must understand, support and participate in the community enhancement initiatives in working towards the vision of Yellow Quill First Nation which improves the quality of life;
4.Work closely with the Health Program Manager and other service providers to improve the health and social outcomes for pregnant women and families with infants and young children;
5.Introduce and assist with the development and implementation of the Focus on Families Framework including policies and procedures in collaboration with the community team;
6.Conduct all tasks with regard to any and all relevant legislation, policies and procedures;
7.Develop culturally appropriate strategies for pregnant women and families with infants and young children based on best practices;
8.Take the lead role in contacting individuals as identified through home visiting;
9.Assist in providing intensive prenatal outreach services to pregnant women using drugs and/or alcohol;
10.Support orientation with pregnant women, their families, and community to explain programs and services available and the program requirements and expectations;
11.Liaise with community service providers, supports and community cultural resources to assist with the implementation of services and supports;
12.Seek additional funding through grants and fund raising activities to enhance the Focus on Families program;
