CORE RESPONSIBILITY:
Provide individual or group assistance and support to individuals with varying abilities, in accordance with their Personal Program Plan and/or Comprehensive Behavior Support Plan, in both facility and community-based settings and in the following areas: development and maintenance of daily living skills, leisure skills, and work skills.
Duties include:
- Provide supportive assistance for the physical/emotional well-being of individuals who attend the Day Program
- Utilize innovation and creativity to ensure all individuals who attend the Day Program are engaged
- Take initiative to de-escalate behaviors ensuring safety at all times
- Provide personal hygiene care and supervise individuals medication when necessary
- Transport individuals to community activities
- Maintain documentation on client data base (computer based) according to agency standards
- Participate in staff training/educational opportunities
Qualifications:
Education:
- Diploma in Rehabilitation Worker, Disability Support Worker Certificate, Special Care Aide, and/or related training and work experience
- Valid First Aid and CPR Certification or willingness to obtain within the first six months of employment
- Valid class 5 Drivers License and working vehicle
- Must be able to work all shifts: days, weekends, and evenings

- Well developed interpersonal skills
- Effective time management, problem solving, and organizational skills
- Ability to work independently or as part of a team
