Description

The Director of Continuing Care and Primary Health Care, in accordance with Saskatchewan Healthcare Management System (Lean) and congruent with the mission, vision and values of the Five Hills Health Region, is responsible and accountable for:

- Overall leadership    
- Quality improvement    
- Strategic planning                    
- Coordination and integration of teams within the Continuing Care and Community Health Service Line.

The Five Hills Health Region has a reputation for leading change and will support the successful incumbent in continuous learning and development.

QUALIFICATIONS    
- Bachelor Degree in Nursing, Masters preferred; registered or eligible for registration with the SRNA.
- 3 years experience in the field/5 years management experience (preferred).
- Lean Leader certified is preferred.

THE FACILITY
The position is located in Central Butte Regency Hospital, Central Butte located at the junction of Hwys 19 and 42, equal distance to from Saskatoon and Regina, and close to Moose Jaw and Swift Current Saskatchewan.
The facility has 22 long term care beds and 7 respite/palliative/observation beds and includes a visiting
physician and nurse practitioner.
