Within the context of a client and family centered model and in accordance with the Saskatchewan Registered Nurses Association (SRNA) standards of professional practice, Code of Ethics, and the established vision and values of the Regina Qu'Appelle Health Region, the Palliative Care Coordinator administers the palliative care registry and priority access to a comprehensive range of palliative care services by establishing and maintaining a single access system for clients diagnosed with a life-limiting illness and their families. This position reports to the Director of Palliative Care Services. The Palliative Care Coordinator in consultation with other team members will have primary responsibility for determining eligibility of clients for the Palliative Care Services Program and its components based on established criteria. The Coordinator provides professional advice to clients with palliative care needs, facilitates regular discussions with clinicians in different settings to determine the most appropriate level of care for clients during different phases of their illness and facilitates access of clients to all components of palliative home care, acute palliative care beds, hospice and long term care.
Baccalaureate Degree in Nursing required; Current practicing registration with the Saskatchewan Registered Nurses Association (SRNA); Valid Driver's License and vehicle; Advanced palliative care nursing certification (CHPCN) recommended. Requires a minimum of three (3) years recent related nursing experience including related palliative care and community nursing experience or an equivalent combination of education, training and experience. Strong background in palliative care setting or hospice desired. Experience in program integration and inter-professional practice desired.
