Description

Under the supervision of the Community Programs Coordinator, or his/her designate, the Community Outreach Caseworker will assist in the development, modification, and the evaluation of the Community Outreach Program as they provide direct services for hard to serve youth in care of the Minister.
Duties:
-Provide stabilization crisis counseling
-Provide incidental problem solving and emotional support to youth
-Implements case plans and monitor success.
-Refer youth to services with other agencies (income support, employment, counseling, addiction services, mental health services)
-Makes or recommends referrals to other professionals as necessary
-Provides information and life skills support to clients related to personal, family, housing, healing, safety, work, school, income, etc.
-Promotes independent problem solving and decision making skills
-To partner with local agencies to develop responsive support for youth
-Provide advocacy services or teach self-advocacy for youth
-Implement emergency, incidental behavior management (non-violent crisis intervention)
-Assist in developing emergency response plan and personal safety plans with individual youth
-Develop and maintain effective working relationships with other agencies
-Provide support, guidance, and direction in a manner that is designated to teach in all aspects of client service
-Recognize potential risks to the well being and safety of self and co-workers and take steps to reduce risk.
-Recognize and assess potential risks to the safety and well-being of youth and the community and take steps to reduce risk and de-escalate situations.

**Must be able to work rotating shifts; shifts are weekday, weekend, day and evening shifts
