  In a 6 week rotation: 12 shifts of 8.00 hours
  
Salary or Pay Band:
  $34.940 to $45.350 (6 step range)

Job Summary:
  The Palliative Care Home Care Nurse is accountable for the day-to-day coordination and case management of all palliative care clients throughout the continuum of care in Meadow Lake, Goodsoil and Loon Lake area. Using a case management model and in partnership with members of the continuum of care, the main function of the incumbent is to establish the direction and coordination of an integrated Palliative Care Program. This position will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters.

Qualifications:
  Bachelor Degree/Diploma in Nursing
  Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
  Valid Class 5 drivers license

Knowledge, Skills & Abilities:
  Ability to adapt to changing conditions
  Ability to travel both within the region and provincially
  Advanced assessment skills
  Advanced communication skills
  Advanced interpersonal skills
  Working knowledge of the RAI-HC Assessment Tool

Additional Text:
  Additional Palliative Care training/experience would be considered an asset.


In an effort to recruit and retain health employees, Prairie North Health Region is offering a package to individuals who have completed training. The package includes $2,500 recruitment bonus with a one year (1,950 hours) return for service commitment; and up to $2,500 for relocation with receipts for an additional one year (1,950 hours) return for service commitment.
