Under the direction of the Director of Health, this position is responsible for the planning, implementation, delivery and evaluation of the Home and Community Care Program. This includes the provision of home care in-home assessments/re-assessments, nursing services, supervision of home care staff, coordination of home health aide service delivery, reporting and accountability for the overall Home and Community Care Program.
PRIMARY AREAS OF RESPONSIBILITY
Program Coordination: supervision of staff, assessments/reassessments, care plan development, client scheduling, referrals, case management, discharge planning;
Home Care Nursing nursing treatments, foot care, injections;
Client/Staff Education;
Records Management;
Medical Equipment & Supplies maintenance;
Reporting & Accountability.
Credentials (certificates, licences, memberships, courses, etc.)
- Licensure as a Registered Nurse by the province/territory of work
- Eligible for licensure as a Registered Nurse by the province/territory of work
