Objective:
The purpose of the Homeward Bound Life Coach is to guide program participants in setting specific, measureable, attainable, realistic and timely life goals. The Life Coach acts as a case manager, a mentor and a teacher to promote self-sufficiency skill building.

Duties and Responsibilities:
Conducts participant intake assessments and develops individualized service plan as appropriate to participant needs.
Links, advocates, and refers participants to community based services as needed.
Facilitates parenting classes, life-skills groups and/or independent living skills sessions as appropriate and needed
Participates in community outreach, recruiting and marketing activities that promote the YWCA Prince Albert and the Homeward Bound program.
Maintains current client and program records, case notes, and other documentation in accordance with industry standards.
Attends multi-disciplinary team meetings as required.
Collects data for and produces outcome measurement reports as required.
Attend provider and other community meetings as appropriate to program.
Maintains current knowledge of community resources as appropriate to program population.
Transports clients on a limited and emergency basis.
Provide short ? term, crisis intervention services.
Monitor clients progress on a weekly basis.
Other duties as assigned.
