Facility: REGIONAL OFFICE (WEYBURN)
Job Description: Reporting to the Regional Manager, Chronic Disease Management, the Chronic Disease Educator is a member of the Sun Country Health Region diabetes team, the kidney health team, as well as a team member for chronic disease programs and services. Key aspects of the job are improving the quality of life of persons with chronic disease in the Sun Country Health Region through the promotion of and provision of education and treatment and serving as a resource to health-care professionals and others to ensure consistent and high quality care for clients with chronic disease by sharing recent advances, knowledge, and experiences in care.
Required Qualifications

Bachelor Degree in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
Minimum of two (2) years nursing experience. Certified Diabetes Educator preferred.
