Description

Duties and responsibilities:  
- Provide one-to-one assistance to clients in job skill training, life skills development, and social skills development as required for maintaining employment
- Develop and implement training plans including task analysis, responsible role modeling and counseling for program participants
- Monitor and participate as necessary to meet production demands at work placement
- Trains at the job site to identify and learn job requirements prior to client starting job
- Build and maintain working relationships with client, employers & staff at worksites and other job coaches.
- Troubleshoot problems that occur in worksites or in daily living skills with clients
- Evaluate & document clients work skills & work habits at each scheduled client visit
- Assist participants with personnel care to promote good hygiene when required
- Support volunteer placements in the community as a means to increase community inclusion
- Promote independence ,self esteem ,and inclusion in the areas of : self care, communication, recreation, socialization, and volunteerism
- Interact with individuals to create meaningful relationships based on mutual interests
- Be committed to support individuals in achieving their goals and dreams according to their person centered plans
- Provide quality programs on-site and engage in recreational and leisure activities in the community including transportation when required  

Qualifications:
- Diploma in Rehabilitation Worker and/or related experience
- Requires valid drivers license and access to working vehicle
- Flexible work schedule
- Valid 1st Aid and CPR Certification or willingness to obtain
- Ability to work independently and as part of a team

The final candidates are required to provide satisfactory police record checks
