Facility: YORKTON HOME CARE
Job Description: The Home Care Manager, under the general direction of the Director of Home Care, is responsible for the overall management of Home Care Program within the geographic assessment area assigned.
  
The Home Care Manager is accountable for management and supervision of the Home Support Services and scheduling in the Yorkton Area.  As a team leader the Home Care Manager will direct the Home Care staff in the most cost-effective manner to deliver quality care.  

The Home Care Manager is also accountable for supervising, evaluating and directing field staff.  Other managerial functions will include buy may not be limited to, assessing client needs, hospital discharge planning, developing and coordinating care plans as well as being the clients advocate.

Required Qualifications

Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
Experience in Home Care.
Experience in administrative or supervisory capacity an asset.
A suitable combination of training and experience may be considered.

Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Excellent leadership, mentoring, organizational, and communication skills
Strong analytical and problem solving skills
Advanced computer skills, including Microsoft Office (Excel, Word and Access)
Ability to be proactive and creative in decision making in the absence of direct supervision
An understanding and commitment to the vision, mission and values of the region
