Under the direction of the Program Coordinator and overall direction by the Executive Director, the Case Coordinator is responsible for the delivery of case management and after care services for the White Buffalo Youth Inhalant Treatment Centre. This also includes treatment planning, case review and aftercare planning and evaluation.
Duties and Responsibilities:
Provide consultation and recommendations to the Program Coordinator relevant to case management and treatment plan review.
Monitor, provides feedback and training on record keeping, consistent with center?s policies with Youth Workers.
Conduct ongoing evaluations of program components to assess effectiveness.
Recommend program and policy improvements and changes based on the evaluation process and evolving information on addiction.
Maintain positive working relationships with outside agencies as it pertains to clinical programs.
Ensure compliance to client commitment to care forms by way of regular contact with referral agents and parents.
Provide regular counselling with clients such as one to one or group sessions to assess client needs.
Assess, plan, direct, implement monitor and authorize service delivery to clients that best meet the needs of the clients within the WBTC mission, vision and values.
Other duties as assigned.
Qualifications & Skills:
Post-secondary degree in psychology, social work, or advanced counselling.
Clinical skills and advanced knowledge of addiction.
Excellent communication and writing skills.
Demonstrated knowledge and experience in assessments, referrals and intervention.
Must be flexible to travel.
Excellent supervision/managerial skills.
