$38.080 to $47.490 (6 step range)
Job Summary:
As part of the Primary Health Care Program this position works in the area of Chronic Disease Management in an outpatient setting to assess, evaluate, teach and monitor clients through a rehabilitation program
(STRIDES). One of the following is required: 1) Cardiac Educator Certificate or diploma through a recognized Canadian College or 2) Pulmonary Educator national certification such as Certified Asthama Educator or Certified Respiratory Educator or 3) Certified Diabetes Educator through the Canadian Diabetes Association. Experience in teaching clients with Chronic Diseases is required. Applicants willing to obtain qualifications within 18 months will be considered.
Valid drivers license required.
Required Qualifications:
Bachelor Degree/Diploma in Nursing
Basic life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association
(SRNA)
