Job Description: Reporting to the Community Team Manager, the Home Care Nurse provides professional nursing care to meet the physical, psychosocial, rehabilitative and emotional needs of the client in a safe and efficient manner, and in accordance with the vision, mission and philosophy of the Prince Albert Parkland Health Region
1.Provides direct nursing care to clients in accordance with policies & procedures and physicians orders as authorized by the Community Team Manager.
2.Applies the nursing process of assessing, planning, providing, evaluating and revising nursing care plans in the provision of care to establish and maintain client independence.
3.Performs client assessments using the formalized assessment format of the Home Care program, and assists in the construction and update of the nursing care plan.
4. Promotes client independence and well-being by supporting, monitoring and teaching skills in self-care to clients and their caregivers.
5. Works collaboratively with other health professionals such as physicians and therapists to meet client and program needs.
6. Provides direction, supervision and support to Home Health Aides and keeps them informed of pertinent client concerns.
7. Teaches, guides and supports family members and formal caregivers in providing therapeutic interventions within the capabilities of the caregiver to ensure safe care and positive client outcomes.
8. Assists in the delivery of Wellness Programs and other health promotion activities as assigned by the Community Team Manager.
9. Deals effectively and promptly with crisis situations by utilizing a collaborative team approach to problem solving.
10. Contributes to quality improvement activities.
11. Participates in the orientation of Home Care nurses and Home Health Aides and may participate in the planning
