Job Description: In collaboration with the multidisciplinary team of the ER, Home Care and the community, the nurse practitioner will provide care for targeted population groups in Prince Albert. This includes: direct patient care, health promotion, disease management and disease prevention to. The position actively collaborates with and refers to other health care professionals; conducts community needs assessments relative to the program mandates and participates in evaluative projects.
Required Qualifications

Baccalaureate Degree in Nursing preferred
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) as an RN (NP)
Experience
Three full time years experience as a registered nurse
Current Level C Basic Cardiac Life Support
Valid driver's license, vehicle and ability to travel
Strong communication and public relations skills
Experience in community needs assessment, community development, and health promotion
2 - 3 years experience as a Primary Care Nurse or Nurse Practitioner preferred


Field hours - 112 hours in 3 week rotation.  Shifts (days, evenings, nights) including actual hours, date of commencement and temporary end dates are for informational purpose only and may be subject to change.
