Facility: COMMUNITY SERVICES
Job Description: Responsible to the Director Primary Health Services, the Community Wellness Coordinator(CWC) will work collaboratively with schools, other intersectoral partners, the Primary Health Care teams and in the community at large to: provide information regarding safe, active healthy lifestyles, facilitate the development of health-enhancement skills in children and youth, facilitate positive decision-making in children and youth, population health promotion addressing the determinants of health, address broader community needs, so that the healthy choice is the easy choice, provide healthy alternatives to drug and alcohol abuse, and build strong community responsible citizens for the future.
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Knowledge of community development
Presentation skills
Ability to apply the principles of community development

  Qualifications

Baccalaureate degree in Nursing, Education, Social Work, Psychology, kinesiology or other health-related discipline.

Minimum of two years relevant experience.

Strong presentation skills.

Knowledge and appreciation of community development principles.

The ability to gain community and peer acceptance.

Ability to function as part of a multi-disciplinary team

Energy that would attract children and young people and an ability to establish and maintain rapport and trust with this group.

Valid drivers license and willing to travel as required.


Geographic Location: KELVINGTON
Expected Start Date: August 29, 2016
FTE: 0.79

Hours of Work: 176.00 hours per 6 week rotation
