Description

Under the direction of the Manager, Settlement Family Support, the Case Management Support Worker facilitates case management team approach that will bring a team of professionals, service providers, programs and individuals to support the client through contribution of resources and expertise in order to avoid duplication, to facilitate communication, to ensure that action plans are carried out and that assessments are consistent and appropriate, team consultations and coordination are enhanced.

**Candidates under consideration will be required to participate in an assessment process consisting of any/all of the following: interview, abilities test, case study and/or presentation.

**Education and Experience:
-Minimum Bachelors degree in social work and 2 years of work experience providing counseling services and case management.
-Must be Registered Social Worker - Member of Saskatchewan Association of Social Workers.
-Cross- cultural counseling experience.
Training in life skills counseling not mandatory but an asset.

**Required Licenses and/or Certificates:
-Canadian Police Information Center (CPIC) certification including Vulnerable Sector Search - no criminal record (done within last three months).
-Holds a valid Class 5 Saskatchewan Drivers License and access to a reliable vehicle for work purposes.
