Job Description: Under the leadership and direction of the Director of Client Services, Home Care South - the incumbent is responsible to act as the primary liaison between Physicians, Facilities, Directors, Health Care Providers and Continuing Care Services (including Access Centre, Home Care, Long Term Care and all Transition Programs - Palliative, Respite, Waiting Placement and Convalescence)
The Assessor Coordinator facilities care planning processes in accordance with Five Hills Health Region Policies, Procedures and Guidelines
Required Qualifications
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work or Licensed Practical Nurse with 4 years experience
Registered or eligible for registration with the appropriate licensing body
Knowledge Skills & Abilities:
- Ability to work with people in a health care environment with professionalism and respect
- Conflict resolution skills to effectively handle difficult issues
- Effective oral and written communication skills
- Excellent assessment, diagnostic and intervention skills
- Good interpersonal skills utilizing sensitivity and courtesy when dealing with patients, staff and physicians
- Knowledge of the health region and community resources
- Leadership skills
- Positive approach and good listening skills to deal with challenging situations
