Job Description:     Accountable to the Manager of Primary Health Care Central, the Chronic Disease Management Facilitator is an integral member of the primary health care team coordinates and facilitates the initiatives and programs involved in the prevention and management of Chronic Diseases within Primary Health Care and the Sunrise Health Region. The Chronic Disease Management Facilitator participates and supports the regional Chronic Disease Management Team in developing their capability to deliver improvements in the care provided to patients with chronic diseases.
The Chronic Disease Management Facilitator promotes quality health care outcomes working within established standards for practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department.
The Chronic Disease Management Facilitator participates in community development initiatives and participates in the evaluation of PHC initiatives within the site.


Required Qualifications

Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Diploma in Nursing with Bachelors Degree preferred
Current BCLS certification
Experience
Knowledge of the Saskatchewan health care system, especially the primary health care system, and an understanding of chronic disease management and health promotion models. Knowledge of Health Quality Council Chronic Disease Management Collaborative Toolkit. Experience in chronic disease/complex illness education or chronic disease management, especially in diabetes, cardiac or renal disease.
