Job Description: A 24/7 position, the Patient Flow Manager (PFM) is accountable for the flow of patients to the appropriate service/facility with the region, and the management and the functioning of the Control Centre on his/her shifts.  The PFM leads discharge planning, and is responsible for the smooth transition of patients throughout the continuum of services in FHHR and other regions where appropriate.  He/She oversees the day to day patient flow and is responsible for effective, efficient utilization of human and physical resources and directs staff to provide safe patient care within these parameters.
Required Qualifications

Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
5 years nursing experience.
Other Information

Knowledge of lean processes would be considered an asset

Expected Start Date: October 31, 2016
FTE: 1.00

Hours of Work: Shift Information: Days, Nights, Weekends, On Call
In a 4 week rotation:  shift of  hour
