Job Description: The MDS Coordinator is a member of the Continuing Care Team who works closely to liaise, consult and support management related to Resident Assesment Instrument/Minimum Data Set (RAI/MDS).  This includes planning, implementation and ongoing evaluation of the programming related to RAI/MDS. May assist the Patient Flow Coordinator in the management of appropriate Long Term Care, Multipurpose Bed and Acute placements to sites within the Region.
Required Qualifications

Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
A minimum of five (5) years current experience in long term care, acute care or home care and demonstrated experience in MDS systems.  Experience in interpreting outcomes and managing resources and projects.  Experience with staff development and training.  Demonstrated sound nursing judgement, initiative and critical thinking.  Demonstrated comprehension of RAI assessment.  Proven consultative and interpersonal skills in order to interact with medical staff, nursing staff, clients and other disciplines.  Demonstrated ability to direct and facilitate change.  Proven ability to manage multiple projects simultaneously.  Demonstrated flexibility, creativity and self-direction.

Ability to competently use computers and software applications as required by the position
