JOB RESPONSIBILITIES:
Effective home care program delivery including complete home care assessments, developing appropriate care plans, and provision of direct nursing care to meet the needs of clients
Indirect supervision and mentoring of other Home Care staff
Record keeping, data collection, program monitoring, reporting and program evaluation
Collaborative, team player
Ability to communicate in Dene would be an asset.
QUALIFICATIONS:
Current registration with the SRNA or SALPN.
Previous Home Care or Community Health experience; must have a minimum of 2 years nursing experience
Clinical knowledge / experience in chronic disease management including diabetes, TB, and pain management.
Knowledge of and sensitivity to Aboriginal cultures and cross-cultural issues
Supervisory and administration skills
Demonstrated oral and written communication skills including computer use
Current CPR certificate Level C
Upon employment must complete:
o WHMIS and TDG certificates
o NITHA Home Care Innoculist exam
o Foot care certification / modalities
Must have a valid drivers license and be prepared to drive on a daily basis
