Description

The MLTC Health & Social Development Department is seeking a qualified Registered Nurse (RN) to fill the full-time position of Home Care Coordinator / Occupational Therapist & Health Benefits Navigator Supervisor.

Guided by the MLFN Community Health Directional Plans and the MLTC Health Development Plan, the MLTC Home Care Coordinator, under the direction of the MLTC Primary Care Programs Manager, will be expected to work with each of the nine Meadow Lake First Nations to continue to work towards and achieve their vision for home care that will include all of the ?Essential Elements? of the Home and Community Care Program.  

The Home Care Coordinator/OT & HBN Supervisor will carry out her/his role by demonstrating effectiveness in the following areas:

-       Community Development; Training-Capacity Building
-        Program Support; Nursing-Home Care Support
-    Policy; Program Reporting Requirements
-    Communication; Advocacy, etc.

Supervisor duties:

-    Support the MLTC Health Benefits Navigator / Occupational Therapist positions through: approvals for leave, program development, mentoring, and work plans, and management of budgets, reporting, evaluation, community support and monitoring of program delivery within the MLFN?s, etc.
