The Community Health Outreach Worker will promote knowledge of and access to programs and services to assist project clients to become and remain engaged in addressing their health status through connection to a community continuum of care. The community Health Outreach Worker will be responsible to the STC Health Centre Director.
STC Health Center offers a coordinated continuum of care to those living with HIV and those at high risk of contracting HIV or STIs. Our goal is to reduce the incidence of new HIV and STI cases in the vulnerable population of Saskatoons inner city neighbourhoods with a special emphasis on youth. The project supports and advances broad-based harm reduction strategies and utilizes a variety of treatment and care options. As part of a multi-disciplinary team and utilizing a variety of outreach and educational approaches, the Community Health Outreach Worker will identify and assist individuals from the target population of Saskatoons inner city to gain access to health services and obtain harm reduction, HIV and STI prevention knowledge. A street outreach and client support to treatment will be a large component of this work.
