DUTIES:
Provides counseling for elderly clients.
Responsible for developing, coordinating, and implementing care plans.
Conducts interviews with clients and their families, to assess physical, mental, and emotional functioning.
Assesses family and social support systems, economic situations, and assists with arranging accommodations and supports if required.
Provides individual counseling for clients and care providers who assist with clients changing health needs.
Provides information about community resources and services which will assist clients in best maintaining health and independence.
Initiates advocacy for clients and their families regarding services and resources.
Schedules home visits for the purpose of assessment, counseling, support, monitoring, and follow-up as required.
Communicates and works co-operatively with Clinic and community health care professionals.
Balances direct service caseload and group programming.
Assesses client population need.
Plans, develops, promotes, facilitates and evaluates assigned groups.
Works co-operatively with the Seniors Volunteer Coordinator who trains volunteers matches volunteers with individuals and groups, and monitors progress and outcomes of the matches
Qualifications:
B.S.W., R.N., or equivalent in training, formal education, and experience.
Must be bondable.
Valid drivers license and a vehicle are required.
