Description

Eden Care is seeking a Full-time Nurse Practitioner to support our communities.
This position works as part of an interdisciplinary health care team.  Under the direction of the Executive Director of Health Services, the Nurse Practitioner will provide health care services for Eden Care Communities. The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for Elders/Residents/Tenants of Eden Care Communities both independently and collaboratively with a physician, depending upon the complexity of needs.  This includes: health promotion and protection; first line treatment; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals.

QUALIFICATIONS    
Certification in a recognized Primary Care Nurse Practitioner program or Nurse Practitioner Masters Program.

Licensed or eligible for SRNA RN (NP) licensure
Minimum of three (3) years experience working as a Registered Nurse, with experience as RN (NP) in a Primary Health Care setting preferred.

- Strong interpersonal and communication skills
- Computer literate in Microsoft Office programs
- Recent experience and demonstrated leadership in the areas of quality improvement, chronic disease management, patient safety, customer service, community development, health promotion and prevention and team based care
