Job Description: The Social Worker-Degree reports directly to the Stroke Services Manager.  The primary function of the Social Worker-Degree is to carry out related duties in accordance with the established policies and procedures.  The Social Worker-Degree is responsible to identify, assess, intervene and follow-up with patients admitted to the Stroke Rehabilitation beds in Jowsey House at the Yorkton and District Nursing Home and outpatient stroke rehabilitation clients in the Sunrise Community.  Social Work services may also be provided to other patients in Jowsey House as caseloads allow.
Required Qualifications

Bachelor Degree in Social Work
Valid drivers license or the ability to travel both within the Region and Provincially
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Experience
One (1) year experience in medical Social Work.
Experience in discharge planning from medical facility.
Experience working with patients affected by stroke.
Other Information

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement
