Description

The File Hills Qu'Appelle Tribal Council is a non-profit organization that provides a range of programs and services to 11 First Nations throughout Southern Saskatchewan within the Treaty Four Territory. Based out of the All Nations' Healing Hospital in the newly constructed Pasikow Muskwa Rising Bear Healing Centre, the Health Navigator will work as part of a multi-disciplinary health care team providing chronic disease and enhanced kidney care services to meet client care needs within a holistic model.

Reporting to the Director of Client Services or designate, the successful candidate will work to enhance the patient experience through the following: advocate for the patient, family and community; utilize cultural and traditional supports in collaboration with the Traditional Knowledge Keeper; bridge communication gaps between all partners and service providers; and implement strategies to address socioeconomic and cultural barriers to care.

Applicants must possess:
-    Recognized health degree (ie. Social Work, Nursing, M.A. Psychology)
-    Current practicing registration with appropriate licensing body
-    Valid Saskatchewan Driver's License

Qualifications:
-    Must be considered to be respectful, approachable and supportive with other agencies to work together in a common goal to wellness
-    Must have understanding and knowledge of programs and services provided in regional health authority, Health Canada, intersectoral services, local communities and the linkages between services
-    Ability to work autonomously and as a member of a team
-    Demonstrated knowledge and respect of, and ability to work within First Nations Traditional and Spiritual Institutes
-    Demonstrated knowledge and experience working with at-risk clients
-    Excellent communication skills, both oral and written, and demonstrated computer skills
