Job Description: The Chronic Disease Management Nurse Practitioner RN(NP) is an advanced practice nurse with knowledge and skills gained through additional education and experience. The CDM Nurse Practitioner in a specialty practice (CDM COPD program) will blend in-depth knowledge of nursing with legal authority and autonomy to order diagnostic tests, prescribe pharmaceuticals and perform specific procedures in the management and treatment of chronic conditions, specifically COPD. This is a dynamic position working in collaboration with an interprofessional COPD team, in a variety of locations and services including hospital, specialty outpatient clinics, and patient homes, and will strengthen working relationships with various health providers. The RN(NP) will participate in continuous program improvements through evaluative processes/projects related to high quality transitions between community and acute care, and improvement of patient flow and connected care for COPD patients. The RN(NP) will provide care within a holistic and culturally appropriate framework for the population served with an understanding of health equity and the impact of poverty and risk conditions on health.
Required Qualifications
Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) as an RN (NP) or RN (GNP)
Experience
3 years as a Registered Nurse required. 1 year experience in chronic disease management, COPD management experience is an asset. 1 year experience in project development, implementation and evaluation using quality improvement methods required.
