Job Description: The incumbent is responsible for developing, implementing, and evaluating specific educational and clinical services, programs for clients, families and health professionals/students in the area of Chronic Disease Management. The incumbent is the lead for the coordination of the dietetic interns experience in CDM and assists in the coordination of the experience for other students. As senior dietitian the incumbent is responsible for the coordination and management of the inpatient and outpatient Pediatric and Young Adult services in the Diabetes Program. This includes nutritional assessment, development and implementation of nutritional care plans, diet counseling to patients and their family members or caregivers, evaluation of the care given, and arrangement for follow-up care as necessary. The incumbent acts as nutrition consultant to other health care team members. The incumbent takes lead on department responsibilities by actively participating in the development of policies, procedures and standards such as the Diabetes Standards Recognition Program and Accreditation. The incumbent participates in quality improvement and research related to the program area(s). Reporting directly to the Manager Chronic Disease Management, the incumbent is self-directed, works independently and within an inter-professional team environment and requires minimal supervision.
Required Qualifications

Bachelor Degree in Nutrition
Accredited Dietetic Internship
Canadian Diabetes Educator Certification Board (CDECB) certification
Master Degree in Health Services Field preferred
Experience
Requires 1 year experience in research in chronic disease management preferably related to diabetes in high risk populations such as pe
