Job Description: Under the direction of the Manager of Rehab and Residential Services the Assessor Coordinator for Mental Health Transitions will be responsible to assist in the successful transition of acute psychiatric patients from hospital into community or alternatively to prevent admission /readmission by providing short term acute mental health in home/community services. Service provision includes: screening, developing care plans, coordination with other service providers and direct mental health support.
Qualifications:
Recognized degree(RN)/diploma(RPN)in Nursing, or a degree in Social Work. Must have current practicing registration with their professional association or the ability to be registered/licensed with their professional association.

72 hours in a 3 week rotation.
