Job Description: Working as an Enhanced Health & Diabetes Centre (EHDC) Team Member, the Behavioral Health Consultant will:
- Provide brief consultation based services to patients, EHDC team members and other health team professionals using an integrated behavioral healthcare model
- Identify, triage, and provide evidence-informed interventions to patients and their families with psychological, physical and behavioral health problems evidenced in the EHDC & primary care settings
- Develop specific behavioral change plans for patients and behavioral health problems evidenced in the EHDC and primary care settings
- Work in a team-based environment to deliver collaborative health approaches and will have considerable clinical autonomy
EXPERIENCE:
- Minimum of 18 months of post-degree demonstrated experience in assessment and treatment of mental and behavioral health disorders or minimum of 1-year practice in providing assessment and therapy in a community based primary care setting working with family physicians.
REQUIRED QUALIFICATIONS
- PhD from an accredited clinical or counseling psychology program
- Registered and in good standing (not under practice restriction or investigation) with the College of Saskatchewan Psychologists
- Possession of valid Drivers License and access to a vehicle is a requirement of the position and condition of employment.
KNOWLEDGE, SKILLS & ABILITIES
- Strong practice management and organizational skills geared to primary care setting
- Knowledge of and ability to apply population-based care principles. Works within a primary care behavioral health model
- Able and willing to work with a diverse population across the entire age span
- History of effectively working in a matrix organizational structure
- Sound clinical and professional judgment and professional judgm
