Facility: COMMUNITY HEALTH/MENTAL HEALTH
Job Description: As Health Educator, you will assist in the planning, promotion and delivery of programs for specific patient groups and individuals.  The focus of this position will be chronic disease management and prevention. You will provide clinical assessment and decision making for cardiac and diabetic education.  Rural travel within the Cypress health Region is required.
Preference given to applicant with CDE or current diabetes edcuation experience
Required Qualifications

Bachelor Degree in Occupational Therapy, Physical Therapy, Social Work, Nursing or Psychology or a related field
Registered or eligible for registration with the appropriate licensing body
Valid drivers license and the ability to travel both within the Region and Provincially.
Experience
Relevant experience which includes experience in health promotion and education.
Experience working in a community setting with interdisciplinary and multidisciplinary teams and the public.
Other Information

Field Hours

Minimum 2 Years Experience in field of practice.  Experience in teaching clients and staff considered an asset  Proficient with the Health Quality Council Chronic Disease Management Facilitation and Toolkit training.  Applicants who are able to complete insulin adjustments is considered an asset.  Preference will be given to applicants who have completed a certificate in chronic disease education or completion of the advanced diabetic program with on going upgrading of knowledge and skills.   The successful candidate is responsible to obtain and provide an original Criminal Record Check (CRC) & Vulnerable Sector Check (VSC) from their local police service, RCMP or previous country/countries of re
