Job Description: COMMUNITY HEALTH NURSE
FOND DU LAC, SK.


APPLICATIONS ARE INVITED FOR A TERM (MAY 2021 TO APRIL 30, 2022) .75 FTE COMMUNITY HEALTH NURSE FOR FOND DU LAC, SASK.  

GENERAL ACCOUNTABILITY
Under the supervision of the Executive Director of Primary Care and the Senior Health Nurse, the Community Health Nurse [CHN] is authorized by the Athabasca Health Authority to function in the role of delivering community health nursing care.  The CHN provides nursing services to clients in communities without hospitals or resident physicians but where physicians work within agreements with the employer to provide medical services on a regular, visiting basis.  The CHN provides a broad range of health care services for the client and the community.  As a member of the Home and Community Care Team, the RN provides direct nursing care, within the RNs scope of practice and under the direction of the Senior Health Nurse of the community.  

DUTIES AND RESPONSIBILITIES
"    Demonstrated ability to function competently as a Community Health Nurse.
"    Cultural awareness and sensitivity to the Dene culture.
"    Interpersonal, organizational and communication skills consistent with establishing and maintaining the working relationships required to effectively perform the role requirements.
"    Ability to work independently and as a member of a multi-disciplinary team.
"    Possess sound judgment, as well as critical thinking skills and problem-solving skills.
"    Ability to communicate effectively and professionally, orally and in writing, with the general public, AHA staff and community members.
"    Ability to prioritize role tasks; react positively to heavy workloads, conflicting demands, and tight time constraints.
"    Assume responsibility and is accountable at all times for personal practice.
"    Demonstrated ability to use standard
