Duties and responsibilities:
The Cognitive Disability Strategy (CDS) is an integrative team planning initiative that offers support to individuals with a cognitive disability throughout the Southeast Region. The Cognitive Disability Consultant (CDC) reports to the provincial team and the Southeast Intake Committee.
- Implements the Principles of Integrated Case Management, including person centred planning and strength-based supports.
- Design, implements, and evaluates behavior support plans in accordance with applied behaviour analysis methodology.
- Provides training regarding behavioural and psychological support strategies and skill deficits to improve knowledge and skills of support teams.
- Works with regional service providers to promote a team approach to services including establishing positive collaborative relationships among health services, social services, justice services and local boards of education.
- Participates on variety of local and regional committees representing the needs of individuals with cognitive disabilities.
- Works with multiple team leads and the Intake Committee to determine eligibility for Flexible Funding Benefit.
- Assists caregivers and family members with the Flexible Funding Benefit application process.
- Coordinates and facilitates community information sessions to promote increased awareness of CDS and serves as a point of contact for all inquiries regarding CDS in the Southeast Region.
Qualifications:
Education:
- A university degree in the human services field, such as Social Work, Education, Psychology, or Nursing.
Experience:
- Experience in behavior assessment, case management to individuals with disabilities and facilitating workshops and working on local/provincial committees.

Final Candidates are required to provide satisfactory criminal record checks including a vulnerable sector search.
