We are looking for a licensed nurse to join our team as Service Needs Coordinator.
The SNC is responsible for completing admission periodic assessments, to ensure care plans for residents are up to date and communicated properly with team members.
They are also responsible for coordinating with CPAS, Home Care, families and other community stakeholders to ensure the needs of our residents are being met.
Hours are Monday to Friday, 40 per week.
