Description

Under the direction of the Fifth Fire Housing Manager, the Cultural Case Manager is responsible for conducting assessments of youth clients who have or will be ageing out of care and arrange, coordinate, and monitor multiple and culturally relevant services from different providers to serve clients needs, especially as they relate to housing.

Duties and Responsibilities
Review client background information, interview clients to assess housing barriers and/or needs of clients experiencing homelessness, prepare intake reports, and manage client cases.
Ensure utilization of relevant cultural resources and practices in addition to assisting clients in locating and utilizing community resources as they relate to housing and other services.
Meet with clients to assess progress, give support, and discuss any difficulties or problems and provide advocacy for higher needs cases as necessary.
Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients overall progress.
Maintain program statistics for evaluation and completion of reporting requirements.
Participate in SIMFC fundraising initiatives, community events, and other duties as assigned.

Education and Experience
Degree in Social Work, Psychology, or other related field; a combination of education and related experience will be considered.
Knowledge and awareness of Indigenous cultures and history.
Strong advocacy skills and knowledge of community agencies and resources in Saskatoon.
Indigenous candidate preferred (please self-identify on cover letter).

Requirements
Strong interpersonal and written/oral communication skills.
Must provide a satisfactory Criminal Record Check with Vulnerable Sector Clearance and Saskatchewan Drivers Abstract.
Must have a valid drivers license and reliable transportation.
