Facility: St Pauls Hospital
Job Description: Reporting to the Manager of Renal Services - Peritoneal Dialysis and Home Hemodialysis, the Patient Educator is responsibilities include: coordination and participation in patient education for chronic kidney disease patients requiring peritoneal dialysis and home dialysis. This will include liaison with families, physicians, dietitians, social work, staff and community agencies, providing individual education for patients and families in a clinic setting, and coordinating and participating in education programs for designated nursing staff that will be prviding peritoneal and home dialysis for patients at SPH and other agencies. The staff member will serve as the primary clinical resource for nursing, medical, and other professional/support staff, participate in departmental activities (program development, planning, quality improvement, and evaluation), research and other projects.
Required Qualifications

Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
3 yrs related clinical nursing experience with patients and families with chronic disease, in particular chronic kidney disease is required.
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Ability to competently use computers and software applications as required by the position
Ability to travel both within the region and provincially
Advanced assessment skills
Communication skills to transfer knowledge/skills
