Job Description: The Assessor  Coordinator will be accountable for providing access and case management services for clients/patients within SHA, including contract tracing for COVID 19. This role will assist the COVID Case Investigator by providing initial notification to exposed persons (contacts) and ongoing monitoring and assessment of contacts.

The Coordinator provides information and facilitates access to a variety of resources, internal and external to SHA.   The Coordinator acts as a liaison and advocate for clients/patients ensuring that those clients with the greatest need have access to service in a timely manner.

Other Information

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.

Required Qualifications:
A degree in nursing, social work, physical therapy, occupational therapy or other Health Services related Degree
Current registration with the appropriate licensing body

Knowledge, Skills & Abilities:
Knowledge of a client centered case management approach
Ability to work with a diverse group of clients
Ability to interact with or work in a team setting
Able to work independently and plan and organize time efficiently
Ability to problem solve and make decisions in a timely manner
Effective communication skills
Demonstrated skill in advocacy, negotiation and conflict resolution
Demonstrated knowledge of community resources, internal and external to SHA
Knowledge of/or willingness to become familiar with medical terminology
Word processing skills necessary
A current valid Saskatchewan drivers license and access to a vehicle in good repair is requi
