The Lighthouse is hiring two Licensed Practical Nurses for our Intensive Support Program. These are full-time permanent positions that average 36 hours per week (one week of 40 hours, one week of 32 hours). Shifts are on weekends and evenings. Responsibilities include screening, assessment, crisis intervention, care planning, consultation, monitoring and follow-up, preparation of reports, data collection, resident, family and staff education. The LPN gives oversight to medication management and with collaboration from medical or clinical professionals, provides on-site support or management of basic health care needs/medical conditions.
