Job Description: The Collaborative Care Outreach Nurse is an advanced clinical practice nurse in the study of Sexual Health and Wellness, HIV, Hepatitis C, Mental Health, Addictions and Community Care Nursing
The Collaborative Care Outreach Nurse requires skill in collaborative multidisciplinary consultation with regional physicians, acute and community based health care services within the PAPHR and other provincial health organizations such as NITHA/FINHB.  The position will ensure intensive supportive care to high risk individuals receiving specialized medical care while remaining in the community.

Required Qualifications

Bachelor Degree/Diploma in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Transportation of Dangerous Goods certification
Valid Class 5 driver's license
Experience
Minimum 2 years direct nursing experience in primary care, public/community health nursing
Preference to nursing experience with sexually transmitted infections and/or blood borne pathogens in a clinical or community setting.
Preference to experience working with clients engaged in at risk behavior and/or marginalized populations

Must be eligibility for clearance to Correctional Facilities

Minimum 2 years direct nursing experience in mental health/addictions and primary care, public/community health or homecare nursing.
Preference to Registered Psychiatric Nurse or RN with minimum 2 years direct nursing experience in an acute or community mental health setting.
